By now, you should have already submitted your Network Agreement which outlines the governance, structure, roles and responsibilities for each practice in your newly formed Primary Care Network.

So, what's next?

The next steps include discussing how your PCN is going to operate and how each practice is going to help undertake the criteria set out in the DES (Directed Enhanced Service).  Your Network will also have to set objectives in preparation for service delivery and maintenance, focusing on developing expanded practice-based and connected teams to deliver the provision of workload and support your member practices.

Cathy Connolly is originally from Northern Ireland where social prescribing activities have assisted with the healing of the country following historical community fractions. Cathy understands the need for a strong community and makes it her mission to go out of her way to help local people. Cathy is an employability expert and has helped hundreds of people back into work. She has excellent links across the region to many different organisations that offer support, help and advice to those in need.

Presenter - Dr Faisal Maassarani, GP Partner, Dr Maassarani & Partners, and Co-founder of Care Meryseside

Dr Faisal Maassarani has experience within primary care in both a provider and commissioner capacity. He is a senior partner in 11 practices within Merseyside which span a total patient list size in excess of 40,000. Dr Maassarani has a specialist interest in the optimal management of long term conditions.


Co-presenter - Dan Hopewell, Director of Knowledge and Innovation at the Bromley by Bow Centre

Dan Hopewell is Director of Knowledge and Innovation at the Bromley by Bow Centre leading on research, evaluation and innovation and development. Through a knowledge share programme, Dan has supported the development of social prescribing throughout the UK and internationally. Previously as the Centre’s Director of Services, he developed one of the first social prescribing schemes in the country. He is currently co-regional lead for social prescribing in London, and a founding member of the National Social Prescribing Network.

Co-presenter - Jennifer Neff, Co CEO and Co-founder of Elemental Software

Jennifer has more than 16 years’ experience in managing public/ private and voluntary, community and social enterprise sector partnerships. She led the delivery of community programmes across employability, digital skills and Active City, age friendly and early intervention.

Co-presenter - Leeann Monk-Ozgul, Co Founder of Elemental Software

Leeann has a long established track record in the community and voluntary sector engaging, activating and supporting our most disadvantaged communities towards better health outcomes. Reducing health inequalities is her passion. Leeann helped establish and manage a Health Living Centre for 15 years in a in Derry City, N.Ireland.
