Innovative new software is being launched which will support the delivery and measurement of the social prescribing model of care at the Health and Care Innovation Expo in Manchester.
Elemental’s Social Prescription Software fundamentally enhances how stakeholders in community health improvement interact to reduce the risk and progression of chronic health conditions. With the cloud based system, Health Professionals and Link Workers are able to generate and, most importantly, track Social Prescription referrals for citizens, connecting them with quality assured providers across the domains of exercise, diet/nutrition, mental health and social support.
‘We had seen first-hand how disjointed health improvement could be’ said Jennifer, Co- Founder of Elemental Software. ‘Commissioners struggled to measure the true impact of programmes and services they had commissioned and the social prescribing approach being championed was presenting a number of challenges. Multi sectoral organisations were often working in separate silos which meant that the citizens who needed the most support were often left behind’.

Elemental’s Social Prescription Software will be launched at Stand K2 at the NHS Health and Care Innovation Expo in Manchester on 7th and 8th of September.
