It’s estimated that around 20% of GP appointments are for ‘non-medical’ needs. In a society that has significant and growing healthcare needs, with detrimental issues such as mental health, loneliness, isolation, obesity and other factors on the increase, growing healthcare requirements are putting a huge strain on NHS resource and ultimately budget.

Connecting those most at risk to non-clinical forms of support through social prescribing can make, and is already making an impact.

Connect stakeholders within the local area through direct referrals

Provide enhanced support by monitoring and tracking referrals

Measure and report on progress and outcomes

Evaluate the impact of social prescribing in your area

Clapham Park Group Practice

“Social prescribing should be available for all GPs to offer their patients. One barrier to universal cover has been the lack of a quality platform for referring, recording and signposting patients, so we’re delighted to be piloting Elemental Software for our partnership with AgeUK.”

- Clapham Park Group Practice GP & Clinical Champion for the RCGP Clinical Priorities Programme, Clapham Park Group Practice
