Radian Housing launch innovative social prescribing project with Elemental
East Hampshire Housing Association, Radian has partnered with Elemental Software to transform the health and well-being of residents through social prescribing.

The new digital project launched last week, and will support and develop Radian’s newly launched Cafe 1759 in Quebec Park in Whitehill and Bordon, one of the UK’s newly named ‘Healthy Towns’ as a social prescribing hub to engage local residents in the wide range of community activities that will be run from the new facility.

Connecting and empowering residents
By implementing Elemental’s Social Prescribing platform, Radian will have the power to link local residents to non-clinical sources of support in their community resulting in:
- Reduced need for clinical treatments
- Ability to engage tenants and residents in accessing community programmes and services that enhance their health and general quality of life
- Personalised social prescribing programmes that suit the needs of their tenants
- Measurement tools to evidence the impact of community investments more effectively and efficiently than ever before
- Boosted staff capacity and strengthened relationships with other key stakeholders
- Opportunity to report on the health and social value of the role that Radian play in alleviating the social and economic burden on the health and social care system in their community
Recent figures from research carried out by University of Westminster show that social prescribing delivers a wide range of health outcomes by empowering people to engage with their own health choices, as well as reducing pressure on the health service by reducing GP consultation rates by 28% and A&E attendance by 24%.

Evidencing and reporting on impact
Radian staff will use Elemental’s Social Prescribing software to directly refer residents to services and support available in Cafe 1959. The platform enables staff to monitor and track the progress of their social prescribing efforts, allowing them to evidence and report on the impact of community health and well-being activities in the community.
