Tech for Good company, Elemental Software, has announced its support for the first North Wales Social Prescribing Conference, which will take place at Theatre Clwyd on 26th October.
The conference is being delivered by 2025, a movement of public, private and voluntary sector organisations in North Wales working together to end avoidable health inequalities in partnership with Betsi Cadwaladr University Health Board.
The conference will launch a “Made in North Wales” approach to social prescribing, a model of healthcare which links individuals to sources of support in their community, prescribing non-medical opportunities to help people adopt healthier lifestyles, which can form part of the solution to improving health and wellbeing in communities across North Wales. The event will see the launch of a North Wales Social Prescribing Network, and aims to engage a wide range of partners from health, local authority, housing and the third sector to meet the needs of the North Wales population.
Margaret Hanson, Vice Chair of the Health Board said: “This conference is a celebration of the excellent partnership work in North Wales between the Health Board and the 2025 movement, and we are delighted that Elemental Software is partnering us in this exciting venture.”
Elemental Software was established in 2013 by social innovators Jennifer Neff and Leeann Monk in response to their work at community and strategic level in Derry engaging communities, healthcare commissioners, employers and health and well being providers to tackle health challenges and create healthy, sustainable communities. Its innovative software enables health and social care providers at all levels, to refer patients , by the touch of a button, to social prescription activities, which could be anything from exercise classes to diet and nutrition courses and mental health support, and to track and monitor their progress through a wide range of different measures.
