Clapham Park Practice Enhance their new Social Prescribing Project Using Elemental Software
We’ve partnered with Clapham Park Group Practice to support and maximise the impact of their new social prescribing project.
Launched in 2015, SAIL – Safe and Independent Living is a social prescribing project run by Age UK Lambeth. Using Elemental Software, ‘SAIL’ community connectors based at Clapham Park Group Practice will now be able to support patients with non medical needs, make direct referrals to local support services including debt advice, befriending groups and home repair services, enabling individuals to live safely and independently in their own homes, and help empower them to reconnect with their community.

Clapham Park Group Practice identified the many benefits of integrating the fundamentals of their new social prescribing project and partnering with Elemental Software to boost the impact.
Implementing our digital platform with the SAIL social prescribing project means Clapham Park Group Practice and Age UK Lambeth can now:

Andrew Boyd, GP at Clapham Park Group Practice and Clinical Champion for the RCGP Clinical Priorities Programme said: “Social prescribing should be available for all GPs to offer their patients. One barrier to universal cover has been the lack of a quality platform for referring, recording and signposting patients, so we’re delighted to be piloting Elemental Software for our partnership with AgeUK.”

Addressing the need for social and reducing clinical
With recent research suggesting that over a fifth of GP appointments are for social needs rather than medical issues, the partnership comes at a vital time. SAIL workers will now be able to signpost those individuals to suitable programmes that will address long term issues, reducing GP workload and waiting times.
