Elemental chosen as digital social prescribing partner for the Hywel Dda University Health Board

Elemental Software has been selected to partner with the Hywel Dda University Health Board. Their ambition is to shift from a service that treats illness to one to keep people well, prevent ill-health or worsening of ill health, and provides support early when needed. Hywel Dda’s hospitals also have a key role in providing quality specialist support when needed, and they want to improve hospital services to establish better outcomes by providing patients with the best standards and safety in care.

 

The roll out of the Elemental system will support the ambition to establish the West Wales region as a connected and kind community.  Across the 3 Counties of Carmarthenshire, Ceredigion, and Pembrokeshire, they will embed their agreed ‘Regional Principles for Community Connectedness and Social Prescribing’ which will enable the effective referral, support, management, and reporting on the social wellbeing needs of the population. Their seven clusters, and range of primary care providers will be pivotal in utilising this support and the Elemental tool will enable a simple referral, communication, and reporting tool.

The main priorities for the project include providing a consistent tool for the whole community of practice to use which supports referrals from multiple sources and a tool to enable the management of the caseload across the system. Through utilising these tools there will be the opportunity to gather and report on identified gaps to enhance their ambition of reducing social inequalities.
