Cwm Taf Morgannwg University Health Board choose Elemental as digital social prescribing partner 

Elemental Software has partnered with Cwm Taf Morgannwg University Health Board and its new Wellness Improvement Service (WISE) initiative to support patients living in Bridgend, Merthyr Tydfil, and Rhondda Cynon Taf to access community activity.

There are considerable challenges to providing healthcare in the wake of a pandemic and the WISE team is aiming to support patients to improve their health.

The adoption of Elemental’s digital platform will enable WISE to support people to access a number of opportunities to improve physical and mental health outcomes, promote wellbeing, and support reducing health inequalities across the entire Health Board area.

“Taking into consideration our holistic lifestyle medicine ethos at WISE and the rapid growth of social prescribing app usage in Wales, we are extremely excited to have partnered with the digital health experts at Elemental.  Our team views health and care apps as a crucial step on the journey to a more patient-centred, self-managed healthcare approach – something close to our heart.

“Social prescribing apps will also help us overcome the current post pandemic challenges we face at Cwm Taf Morgannwg, and with the future in mind, provide an alternative healthcare approach to our population with ever increasing life expectancy and more long-term conditions.”

 

In addition, WISE aims to assist in offering alternative non-medical intervention support to patients. It will focus initially on four separate cohort conditions and will target patients who are currently on secondary care waiting lists for treatment. Liza adds:

“All our patients will be be allocated a WISE wellness coach who will deliver a supported patient self-management schedule over nine months to assist them to reduce their burden of illness.

“Our team is skilled in supporting change and can offer our patients a variety of opportunities such as social prescribing, walking programmes and individual symptom management, all within a holistic lifestyle medicine framework.”

 

Through the social prescribing platform, Cwm Taf Morgannwg University Health Board’s WISE team aims to:

  • improve patient’s health outcomes while waiting for treatment;
  • improve physical and mental health outcomes;
  • reduce health inequalities across the Health Board area;
  • promote wellbeing;
  • and improve access to health improving support.
