Elemental celebrate Link Worker Day 2021

Every year, we at Elemental celebrate Social Prescribing Link Worker Day by
shining our spotlight on our customer successes, achievements, and
milestones. This very special day recognises Link Workers who go above and
beyond to make a difference in people’s lives, have persevered through
challenges, and have connected people into their communities. Today, we
share with you, a special customer focused selection of Link Worker resources.

Earlier this week, we caught up with Sue Pomfret, Social Prescribing Officer and Abby Turner, PCN Link Worker from Action Together to learn more about their roles within the community. Elemental and Action Together share with you, a series of questions and answers and patient stories.

The Rainbow Centre is a charity that provides support, services and facilities that empower people of all ages to retain independence in the localities of Wrexham and North Shropshire. Jo Young, Senior Social Prescriber and Wellbeing Lead at the Rainbow Centre Penley, tells us what she loves about her job!
