East Cambridgeshire Integrated Partnership (ECIP) has teamed up with Elemental to create a joined-up social prescribing service for local people. ECIP is a collaboration between Ely North and South Primary Care Networks, East Cambs District Council, How Are You Peterborough, Cambs County Council, and other statutory and voluntary community services.
The two Primary Care Networks include six GP practices supporting over 75,000 patients across rural East Cambridgeshire. Having recruited a number of new roles including Social Prescribers, Health and Wellbeing Coaches and Care Co-ordinators, and with the area’s innovative approach to community projects and place-based services, the PCNs wanted to invest in a digital system to enable local partners in East Cambs to easily refer patients, share information, and join-up services.
“We’re delighted to be working with Ely North and South Primary Care Networks to support them in taking this exciting project forward that will help to transform the way social prescribing is delivered locally. The project will allow partners to integrate our digital platform with GP clinical systems, as well as allowing patients to self-refer. Crucially, partners will be able to measure the progress of patients on their social prescribing journey and track their health outcomes, so that they can really demonstrate the impact of services on people’s lives.”

We see the self-referral module as a step towards improving access to support, and a step towards reducing inequalities. We like the function whereby Social Prescribing work is automatically coded in a patient’s clinical records. We’ve appreciated the support from the Elemental team in taking this project forward.”
In joining up with Elemental, the integrated partnership aims to enable up to 2,000 social prescribing referrals a year.
