BETSI CADWALADR UNIVERSITY HEALTH BOARD IS COMMITTED TO HELPING BRIDGE THE GAP BETWEEN SECTORS, SERVICES, AND THE COMMUNITY SO THAT MORE PEOPLE ARE ABLE TO ACCESS THE NON-MEDICAL SUPPORT THEY NEED, WHEN THEY NEED IT AND HOW THEY NEED IT.

Mantell Gwynedd is the most recent project in North Wales to adopt Elemental’s digital social prescribing technology in a bid to enhance the reach of their social prescribing scheme in Arfon, Gwynedd.

The scheme utilises the social prescribing model as an effective early intervention method to identify and encourage people to take control of their own life to make changes that improve their health and emotional well-being. A social impact assessment (SROI – Social Return on Investment) has been undertaken of the project on a continuous basis since 2017.

The social prescribing scheme was set up to:

Embedding digital into the social prescribing scheme via Elemental’s Core platform will embed a clear referral pathway and make it easier to connect people to classes suited to their needs, and will also enable Mantell Gwynedd to:

“We’re really excited to be partnering with the team at Mantell Gwynedd due to their commitment to improving wellbeing and quality of lives being tightly aligned to ours. The on-going social impact assessment will greatly benefit from the reporting capabilities within Elemental, and give them concrete evidence on the impact of social prescribing in Arfon as well as help to identify gaps in service provision. We’re looking forward to seeing the impact reports that will no doubt showcase the fantastic benefits of this scheme and social prescribing.” – Jennifer Neff, Elemental Co-Founder, and CEO

Jennifer and Leeann

Someone’s health and wellbeing can be affected by where they live, what they do for a living, their income or their early childhood background experiences. This in turn leads to some stark, and often avoidable health differences.

We founded Elemental to play an active part in halting health inequalities through the social prescribing movement. Our technology helps communities to be better connected, build resilience and bring real precision to the measurement of the impact of community investment.

We work with organisations that want to continue to invest in their communities but want to be much better at measuring impact and outcomes.

We believe this is a powerful route out of health inequality.
