Elemental’s Self Refer is empowering people by making it easier for anyone to instantly request support during Covid-19

ELEMENTAL’S SELF REFER TOOL IS DESIGNED TO PROMOTE A CULTURE OF SELF CARE AND INDEPENDENCE, WHICH CAN EASE PRESSURES ON GPS AND LINK WORKERS DURING THE CURRENT PANDEMIC AND BEYOND. CITIZENS AND PATIENTS CAN NOW INSTANTLY REQUEST SUPPORT OR CONNECT WITH A LINK WORKER ONLINE, AVOIDING THE NEED TO WAIT TO BE CONTACTED OR BECOME FRUSTRATED WHEN FACED WITH CHALLENGES AROUND MAKING CONTACT WITH GPS OR HEALTHCARE PROFESSIONALS.
HOW IT WORKS

OPTION 1

A button can be added to your existing website which links directly to a Self Refer webform.

OPTION 2

The Self Refer form can be directly embedded on an existing page.

This allows citizens to register themselves instantly to speak with a Link Worker or request support. This form/page is responsive and will flex to fit on any screen size on any device e.g tablet, kiosk, phone, laptop.

Organisations in health, housing, VCSE and Local Government can add their own logo, page title and description, allowing brand continuity and personalisation around messaging.

AFTER A REFERRAL IS MADE, THE FOLLOWING STEPS ARE CARRIED OUT:

 Step 1

A Link Worker makes an initial call to the citizen/patient to get an understanding of the citizen’s/patient’s existing connections, general wellbeing and specific needs. Baseline assessments are carried out at this stage.

 Step 2

A support package is co-created and introductions are made to community support/ providers.

 Step 3

Frequent follow up calls are made to check in on how the patient is feeling and offer extra support if needed.

All referrals made through the website feed through into the Elemental’s Core Platform, meaning you can manage, track, measure and report on the number of referrals received, drill down on reasons for referral, location, and a number of data points. 

This also means you can actively track and measure the impact of your support services and interventions.

SELF REFER IN ACTION

In a bid to empower people and give them independence over their own care and support during Covid-19, Elemental customers Care Merseyside and the Old Library Trust (OLT) recently began trialing Elemental’s Self Refer tool.


By visiting the OLT or Care Merseyside websites, local citizens, residents and patients can instantly register themselves to speak with a Link Worker or request social or wellbeing support.

Since going live, Care Merseyside saw a staggering 38 referrals coming through their website in just 24 hours.

“Our Elemental Self-Refer Hub went live yesterday and Dr Massaranni & Partners GP Surgery sent a text to their patients with a link to service.
