Elemental appoint The Bureau as their digital social prescribing training partner for Greater Manchester Health and Social Care Partnership contract

Big Help Project partner with Elemental in a bid to support successful client reablement ‘beyond the gate’
LAST MONTH ELEMENTAL WERE ANNOUNCED AS THE PREFERRED SOCIAL PRESCRIBING IT PLATFORM PROVIDER TO ENHANCE THE ROLL-OUT AND IMPACT OF SOCIAL PRESCRIBING ACROSS GREATER MANCHESTER ON BEHALF OF THE GREATER MANCHESTER HEALTH AND SOCIAL CARE PARTNERSHIP.

In addition to the supply of the platform itself, Elemental offer a suite of services that include Link Worker training, integration with clinical systems to enable referral making, managing and reporting, and also the ability for people to self refer themselves to social prescribing Link Workers. 

Elemental wanted to provide digital social prescribing training and user training at a hyper-local level and also wanted to form a strategic partnership with a local VCSE organisation to support the roll-out of the platform. That’s why The Bureau has been recently appointed by Elemental to manage the training element of the contract.

BY EMBEDDING ELEMENTAL’S DIGITAL PLATFORM INTO EXISTING SOCIAL PRESCRIBING PROGRAMMES ACROSS GREATER MANCHESTER, NHS ORGANISATIONS AND COUNCILS WILL BE ABLE TO:
  •  Ensure the effective roll-out and widespread usage of social prescribing across Greater Manchester localities
  •  Collect all monitoring data needed for robust evaluation
  •  Pull individual data from existing records and push back information into the patient record
  •  Make it easier for commissioners and providers to demonstrate the effectiveness of social prescribing interventions
  •  Provide referrers with useful real time information on progress with the referral after it has been made
  •  Collate data at a granular neighbourhood level but also provide easy access standardised data around social prescribing at a Greater Manchester level
  •  Track progress on outcomes, via standard and bespoke reporting, for individuals, localities and Greater Manchester as a whole

Greater Manchester Health and Social Care Partnership will strengthen the footprint around which integrated community based teams will develop in a bid to tackle and treat the wider health needs of their local communities.

The Bureau is a nationally accredited volunteer centre who are committed to developing solutions that can be delivered through effective voluntary action and partnership working.

The Bureau, founded as Glossop Volunteer Bureau almost 40 years ago in 1979 to establish much needed services for local people across Glossop dale that could be developed and run by volunteers, has grown in recent years to cover a whole range of new support projects, sessions and services.

The Bureau manages the delivery of social prescribing services in Glossop on behalf of Tameside and Glossop Integrated Care Trust and Glossopdale PCN.
