WEST LANCS CCG PATIENT FOCUS: COLLABORATING THROUGH SOCIAL PRESCRIBING TO REDUCE BLUELIGHT CALLS BY 92%

By working together and liaising with other health and social care colleagues Skemersdale Social Prescribing team have been able to reduce calls from the patient to the ambulance service by 92% and subsequently the patient has improved a positive shift in her physical and mental wellbeing.READ MORE

The Merton Voluntary Service Council Social Prescribing Coordinator (SPC) delivered the hugely successful pilot through two GP Practices,Wideway Medical Centre and Tamworth House Medical Centre. This year we also welcomed the announced around the  forming of PCNs and investment in Link Workers. Whilst the adoption of social prescribing continues to evolve, we are committed to ensuring that social prescribing works for everyone in order to connect people, building communities and improve lives. 

We’d like to say a massive thank you to all of our team, customers, supporters, advisors and investors for their contribution to our vision in 2019.

We’re ready to take digital social prescribing to the next level in 2020 through further innovation and interoperability capabilities on our platform, detailed and quantifiable evidence and case studies from our customers, wider personalised care thought leadership, further growth in our team and more opportunities for social prescribing champions to collaborate and co-design their social prescribing projects together.

Someone’s health and wellbeing can be affected by where they live, what they do for a living, their income or their early childhood background experiences. This in turn leads to some stark, and often avoidable health differences.

We founded Elemental to play an active part in halting health inequalities through the social prescribing movement. Our technology helps communities to be better connected, build resilience and bring real precision to the measurement of the impact of community investment.

We work with organisations that want to continue to invest in their communities but want to be much better at measuring impact and outcomes.

We believe this is a powerful route out of health inequality.
