The Wellbeing Prescription is delivered in partnership by Tandridge District Council and Reigate & Banstead Borough Council and is commissioned by NHS East Surrey Clinical Commissioning Group and Surrey County Council Adult Social Care.The service works closely with all of the GP practices in the area, local voluntary and community organisations and Surrey Public Health.
After recognising the benefits of digital social prescribing and in a bid to enhance the effectiveness of their existing Wellbeing Prescription and reporting ability, Tandridge District Council has embedded Elemental’s Core platform.

The difference that Wellbeing Prescription is making to people’s lives is measured on the impact on the person taking part in the programme, the impact on the community and the difference made to the health and social care system and Programme Manager, Liz Carter, wanted to ensure that the programme was achieving its aim of supporting people to identify, set and meet achievable goals and signpost people to local services which they feel would benefit their health and wellbeing.

Wellbeing Advisors work out of all of the GP surgeries in the East Surrey CCG area which are the foundation of the programme. Patients registered with these GP practices will now be offered the opportunity to become better connected into their communities and more empowered to tackle a range of issues including mental health, diabetes, domestic abuse, alcohol addiction, dementia and bereavement through community programmes.

Wellbeing Advisors from the programme come from a wide variety of backgrounds ranging from nutritionists, nurses, personal trainers and psychotherapists.
Referrals into the Welling Prescription can come from Health and Social Care professionals and from patients themselves. Professionals making referrals can then track the progress and view follow-up notes for each.
Wellbeing Advisors will communicate with programme participants to establish how a they’re feeling and co-create a plan with goals and actions to help them overcome any challenges they are facing in their life.
Community Providers like YMCA, the Befriending Service and Men in Sheds, will receive referrals via Elemental’s Core Platform and will be able to check participants in to classes and sessions, providing a better understanding of the demand for community based programmes and services.
By connecting up all the different stakeholders and assets in the community a more encompassing service can be provided to the people of Tandridge and Reigate & Banstead areas.
