SPRING Social Prescribing Programme Bridges the Gap Between Health Care and the Community

LAST YEAR, ELEMENTAL WAS ANNOUNCED AS THE SOLE DIGITAL PARTNER OF THE SPRING SOCIAL PRESCRIBING PROGRAMME, A JOINT PROJECT BETWEEN THE HEALTHY LIVING CENTRE ALLIANCE AND SCOTTISH COMMUNITIES FOR HEALTH AND WELLBEING WHICH WAS SUCCESSFULLY AWARDED MORE THAN £3 MILLION OF NATIONAL LOTTERY FUNDING.

CONNECTING TO COMMUNITY SUPPORT

Since then, Elemental has been working in partnership with the Healthy Living Centre Alliance and Scottish Communities for Health and Wellbeing to train Social Prescribers on our digital social prescribing platform, which connects individuals to non-clinical support in their community, including cookery classes, mindfulness, exercise activities and gardening groups.

Via the simple referral pathway, Social Prescribers across the Healthy Living Centre Alliance and Scottish Communities for Health & Wellbeing receive referrals to their programmes and services from GP practices, reducing avoidable appointments, cutting waiting times and supporting individuals to take greater control of their own health and well-being.

A MORE THAN MEDICINE APPROACH

The social prescribing partnership, which is the largest coordinated project of its kind is expected to receive over 16,000 social prescribing referrals to 17 Hubs in order to tackle isolation and dependence on medication in 30 disadvantaged areas in Scotland and Northern Ireland.

Accessing Elemental’s Core platform, clinicians and Social Prescribers are able to monitor and track referrals, allowing them to provide enhanced support for the patient, log progress and build a case file.

Over the next 5 years, the SPRING Social Prescribing Programme will coordinate 96,000 community based interventions and support over 8,000 individuals in their health improvement journey.

SUPPORTING NATIONAL STRATEGIES FOR HEALTH IMPROVEMENT

For the first time, the Healthy Living Centre Alliance and Scottish Communities for Health & Wellbeing will also be able to accurately report and evaluate the impact of their community based care, providing the evidence it needs to secure further funding, demonstrate social value and inform how they shape their future social prescribing programmes and services.

Our Social Prescribers will record everything from phone calls, meetings, engagement in activities to outcomes achieved, tracking the patient’s health journey. Elemental will be vital in allowing us to demonstrate the impact on community led health approaches to Scottish Government NHS and other key stakeholders and hopefully embed this model into national strategies for health improvement.”
