Funded by the Health Services Executive, Healthy Ireland and managed by the South Dublin County Partnership, the pilot project will address people’s needs in a holistic way and empower individuals to take greater control of their own health.
Using Elemental’s Core platform, GPs within Glenview Clinic and Park House Family Practitioners will be able to make instant social prescribing referrals to South Dublin County Partnership, reducing avoidable GP appointments while also helping patients make positive lifestyle changes.
Following the referral, GPs can also track the status of referrals at different stages, providing enhanced care enriching the conversations they have with people when they come into surgery.
South Dublin County Partnership will now have the ability to access, report and showcase detailed data analytics which helps to shape their social prescribing services and close the loop between clinicians and the community.
Using these metrics, South Dublin County Partnership can demonstrate the impact that their social prescribing programmes and services are making to their community, tackling a key challenge of VCSE organisations.
With almost 1,300 people a day failing to attend outpatient appointments nationally, medical practitioners in two GP practices in South Dublin will be offering patients the opportunity to avail of a social prescription and engage in appropriate community support and activities.
