South Dublin County Partnership Take A Holistic Approach to Community Centred Care

SOUTH DUBLIN COUNTY PARTNERSHIP, A LOCAL DEVELOPMENT COMPANY IN SOUTH DUBLIN COUNTY HAS LAUNCHED AN INNOVATIVE PARTNERSHIP WITH ELEMENTAL WHICH CONNECTS PATIENTS TO SOCIAL PRESCRIBING ACTIVITIES SUCH AS ART CLASSES, WALKING GROUPS, BEFRIENDING SERVICES AND EMPLOYMENT ADVICE.

Funded by the Health Services ExecutiveHealthy Ireland and managed by the South Dublin County Partnership, the pilot project will address people’s needs in a holistic way and empower individuals to take greater control of their own health.

CONNECTING CLINICAL AND COMMUNITY RESOURCES

Using Elemental’s Core platform, GPs within Glenview Clinic and Park House Family Practitioners will be able to make instant social prescribing referrals to South Dublin County Partnership, reducing avoidable GP appointments while also helping patients make positive lifestyle changes.

Following the referral, GPs can also track the status of referrals at different stages, providing enhanced care enriching the conversations they have with people when they come into surgery.

South Dublin County Partnership will now have the ability to access, report and showcase detailed data analytics which helps to shape their social prescribing services and close the loop between clinicians and the community.

Using these metrics, South Dublin County Partnership can demonstrate the impact that their social prescribing programmes and services are making to their community, tackling a key challenge of VCSE organisations.

PRESSURES IN GENERAL PRACTICE

With almost 1,300 people a day failing to attend outpatient appointments nationally, medical practitioners in two GP practices in South Dublin will be offering patients the opportunity to avail of a social prescription and engage in appropriate community support and activities.

“We are really looking forward to linking people to opportunities in the community that can improve their health and wellbeing. There is so much people can do in their own area – they sometimes just need a little help to make the connection. The software will support that through secure referrals and key reporting data, which is key in gaining the trust of our healthcare partners and funders.” Marjolein Moonen, Deputy CEO at South Dublin County Partnership

EMBRACING DIGITAL SOCIAL PRESCRIBING TECHNOLOGY

Enhancing their social prescribing programmes using digital social prescribing technology, means South Dublin County Partnership can now:

  • Save time and resource by reducing administrative pressures on staff
  • Create personalised social prescribing programmes for individuals
  • Connect key delivery partners using one central platform
  • Report on project outcomes and provide detailed evaluations for stakeholders
  • Track and monitor the impact of social prescribing referrals

Jennifer and Leeann

Someone’s health and wellbeing can be affected by where they live, what they do for a living, their income or their early childhood background experiences. This in turn leads to some stark, and often avoidable health differences.

We founded Elemental to play an active part in halting health inequalities through the social prescribing movement. Our technology helps communities to be better connected, build resilience and bring real precision to the measurement of the impact of community investment.

We work with organisations that want to continue to invest in their communities but want to be much better at measuring impact and outcomes.

We believe this is a powerful route out of health inequality.
