Who are we?

Elemental co-founders, Leeann Monk and Jennifer Neff, saw first-hand how disjointed public health improvement could be despite the best efforts of stakeholders. Leeann managed a Healthy Living Centre in one of the most socially deprived parts of Northern Ireland for 13 years and Jennifer led the citywide coordination and delivery of the Active City strategy for Derry.

Our experience in resourcing, designing and delivering community based health improvement programmes led us to see that despite millions of pounds being spent on health improvement, the message was often not reaching those most at risk. Ultimately, this meant that those who needed the most support, were often left behind.

Our team has grown now to include specialists in social prescribing, lifestyle behaviour change, diabetes prevention, patient activation and enterprise software development.

We have recently secured partnerships with Local Authorities, Clinical Commissioning Groups, Community and Voluntary sector organisations and Housing Associations to take their social prescribing programmes to the next level.
